How to obtain a medical marijuana document (prescription)
Region: Ontario Answer # 2482You must qualify to legally purchase cannabis for medical purposes. The first step is to obtain a medical document from a licensed medical practitioner (physicians and, in some provinces, nurse practitioners) who is responsible for treating your condition and symptoms. The document will detail exactly how much you can use and how long the treatment period will last.
How to obtain a medical marijuana document
- Meet with your health care practitioner to discuss whether medical cannabis could treat your symptoms. To determine this, they will review your medical history and may conduct an appropriate physical examination.
- If your health care practitioner decides to prescribe medical cannabis as a treatment for your condition, you will be asked to sign a “treatment agreement” outlining your medical and legal responsibilities with regards to your prescription. For example, the patient agrees to use cannabis as prescribed, will store their cannabis in a safe and secure manner, and will not sell or give away their cannabis.
- Your health care practitioner will complete and sign the medical document(s)
What must be included in a medical marijuana document?
Section 273 of the Cannabis Regulations states that a medical document must include:
(a) the health care practitioner’s contact information: name, profession, business address, telephone number, fax number and email address;
(b) the province in which the health care practitioner is authorized to practise their profession and their licence number;
(c) the patient’s contact information: name, date of birth;
(d) the address of the location at which the individual consulted with the health care practitioner;
(e) the daily quantity of dried cannabis (in grams), that the health care practitioner authorizes for the individual; and
(f) a period of use, such as the number of days, weeks or months (cannot exceed one year)
The document must be signed and dated by the health care practitioner who is providing it. It must also include a statement confirming that the information in the document is correct and complete.
